We are hiring a full-time Physical Therapist to join our dynamic care team at Cape Heritage Rehabilitation & Health Care Center in Sandwich!

Role
and Responsibilities
Provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload.Participate in the clinical team process and provide teaching and training to caregiver staff.Supervise other therapists, assistants, aides, and students as required.Screen and evaluate patients/residents and develop appropriate care plans.Perform administrative duties such as overseeing resident care activities, managing medical records, and ordering supplies.Provide direct resident treatment under a physician’s order, including individual, supervised, and group sessions.Contribute to team meetings and decision-making processes.Document activities accurately and timely according to the company’s system.Ensure compliance with Medicare and other reimbursement regulations.Communicate effectively with patients, residents, team members, and supervisors.Participate in clinical program development, staff training, and orientation.Assist in census building and marketing activities.Engage in quality improvement activities.Ensure service and operational excellence in all activities.Protect residents’ health information and adhere to privacy policies.Exhibit courtesy, compassion, and respect to all stakeholders.Minimum Requirements
Bachelor’s, Master’s, or Doctoral degree in Physical Therapy.Appropriate licensing or in the process of obtaining one.Effective communication and organizational skills.Ability to perform documentation, equipment manipulation, and direct patient care.Ability to handle stress and prioritize tasks effectively.Ability to work with diverse populations and backgrounds.Physical ability to stand and walk for extended periods.About Athena Health Care Systems
