An exciting opportunity has arisen for a registered nurse to join our friendly and forward thinking community care team in SMASH.

This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home.

As part of a team you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes and the delivery of high quality holistic care within the community setting, and within our wound care clinics. You will be expected to support your peers, the team leader and the organisation in promoting the Trusts values and behaviours.

The hours of work are between the hours of 08:00 and 17:00, over 7 days.

You will need to be able to work independently, possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas if required to meet service need.

Please note whilst main base will be at Alsager, you will be required to work across the footprint of CCICP

Main duties of the job

We are looking for a Band 5 staff nurse to support the Caseload holders in delivering high quality, efficient and safe care.

We are looking for an enthusiastic, person who possesses both clinical and digital skills to support in the promotion of quality improvement for a local population.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. Job responsibilities

To work under the direction of the Community Nurse Team leader and to be responsible for the assessment and management of the health needs of an identified population and for the development, implementation and evaluation of programmes of care within range of target groups including end of life care and patients with long term conditions and complex nursing needs.

To support, and wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life. To be expected to hold a designated caseload and deputise in the absence of the District Nurse.

To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery.

To work within the agreed framework for Community Nursing Services.

To assess, provide and review holistic nursing care, advice and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence based knowledge.

To undertake initial assessments on receipt of specific referrals to the caseload, acknowledging the needs of clients and their carers.

To assess, plan and implement and evaluate a plan of care to meet the patients physical, social, emotional and environmental needs, encompassing a rehabilitative/ self approach where necessary and as per district nursing contract.
