The Registered Nurse provides, coordinates, and directs home nursing care to patients according to physician orders and in alignment with agency policies and procedures. Care is delivered through the competent application of the nursing process, ensuring high-quality, patient-centered services in the home setting.
The RN is responsible for:
- Assessing, planning, implementing, and evaluating patient care.
- Supervising Home Health Aides and Licensed Practical/Vocational Nurses (LPNs/LVNs).
- Reporting to the Clinical Manager.
- Perform comprehensive assessments of patients' medical conditions and needs.
- Develop individualized plans of care, update them as patients' conditions change, and document all clinical findings accurately.
- Provide skilled nursing services, including medication administration, wound care, and disease management education.
- Monitor patient progress and communicate significant changes to the physician and care team.
- Supervise and evaluate Home Health Aides and LPNs/LVNs in the delivery of care.
- Educate patients and families on health conditions, treatments, and self-care strategies.
- Maintain compliance with professional standards, state and federal regulations, and agency policies.
- Coordinate care services and serve as the primary point of contact for patients and families.
- Complete timely and accurate clinical notes and progress reports.
- Regular home visits within the assigned service area.
- Attendance at periodic office meetings.
- Reliable transportation, a valid driver's license, and agency-required automobile liability insurance are required.
- Current, active, and unencumbered Registered Nurse license in the state of practice (or multi-state license under the Nurse Licensure Compact, if applicable).
- Minimum of 1-2 years of nursing experience, preferably in home health or acute care.
- Current CPR certification.
- Strong assessment and critical thinking skills to identify and respond to patient needs.
- Competency using computers, including electronic medical records, email, and online training systems.
- Ability to develop and implement effective care plans.
- Compassionate communication and education skills for patients and families.
- Organizational skills to manage multiple cases and documentation requirements.
- Professionalism and ethical conduct in all patient interactions.
Pay Rate is dependent on seniority and other factors that will be discussed during the hiring process
