Qualicare delivers compassionate home care and home health services across the Bay Area. We are expanding our team in San Ramon and seek a dedicated Care Coordinator to manage client care plans, schedule caregivers, and maintain quality of service.

• Coordinate client intake, assessments, and care plans, ensuring documentation is accurate and up-to-date.
• Match caregivers with clients based on skills, experience, and availability, adjusting schedules to maintain coverage 24/7.
• Respond to last‑minute schedule changes, call‑outs, and urgent staffing needs, providing orientation and ongoing support to caregivers.
• Serve as the primary contact for clients and families, communicating scheduling information, service questions, and care progress.
• Track and report key metrics, including client satisfaction, missed shifts, and service hours.
• Maintain compliance with state regulations, agency policies, and documentation standards.

• High school diploma or equivalent required;
  Associate’s or Bachelor’s in healthcare administration, nursing, or social services preferred.
• 1–2 years in home care, healthcare, case management, or scheduling/coordination roles.
• Experience with caregiver scheduling systems (Clear Care/Alora/Matrix Care) a plus.
• Strong organizational and time‑management abilities; excellent communication and problem‑solving skills.
• Proficiency with Microsoft Office and scheduling software.
