Lead Case Manager, Permanent Supportive Housing (PSH) open position. About St. Vincent de Paul Society Of Marin (SVdP):

Responsibilities- Assess clients (using the MAV-SA or other assessment tool) and document housing needs, strengths, resources, cultural identity, behavioral health, medical, social, family support, education and employment history.
- Partner with clients to create individualized housing and treatment plans; document interventions in case notes and HMIS/WZRD systems.
- Develop and provide, or connect to, high-quality wellness and recovery interventions and services.
- Provide strength-based case management, medical care support, therapy, crisis intervention, and peer/family support.
- Document case notes and client-related expenses in required database systems.
- Determine the type and intensity of services needed to help clients achieve housing stability, including check-ins, transportation, mental health and/or substance use considerations, and stabilization services.
- Provide housing location services: assess unit characteristics, locate units near transit, network with landlords, and accompany clients to housing-related appointments.
- Assist clients in securing personal documentation and required paperwork, including reasonable accommodations for rental subsidies or housing vouchers.
- Assist clients in moving into housing and setting up basic household items as needed.
- Provide housing stabilization services and support clients in maintaining tenancy, navigating finances, and coordinating with landlords.
- Collaborate with clients to address tenancy issues early to retain housing, including boundaries, payments, and lease compliance.
- Meet with clients weekly at move-in and regularly thereafter; visit tenants at home at least once per month to discuss progress and challenges.
- Maintain monthly contact with the client’s housing property manager to address housing challenges and provide tenancy training.
- Independently supervise and manage 2–3 PSH Case Managers with leadership and supervision of the team.
- Provide space for individualized supervision (one-on-one) weekly for each case manager under the Lead’s supervision.
- Support other PSH Case Managers with guidance, case conferencing, time accounting, car reservations, and administrative tasks as needed.
