Position
:
Full Time Registered Nurse Case Manager (RN) with part-time opportunities available. Territory:
Marin County.

Responsibilities
  • Utilize clinical skills to create, implement, and execute the plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
  • Oversee the assistance and teaching of patients and family/caregivers for wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, CHF management, and medication education in the patient’s home.
  • Perform a full assessment of the patient and family at initial visit and develop an advanced plan of care including rehabilitation.
  • Work with patients and family one-on-one to achieve improvement and/or stability.
  • Case manage the patient’s care episode with an interdisciplinary clinical team.
  • Prepare clinical notes, orders, and charting in a compliant time frame.
  • Collaborate with physicians and agency staff to deliver the best care possible.
  • Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.
Qualifications
  • Current and unrestricted CA RN license
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • High level of clinical knowledge in home health care is highly preferred
  • Experience with high-acuity patients is a plus
  • Experience working with an interdisciplinary team is highly desired
  • Case Management fundamentals is highly desired
  • Previous experience with IV medication infusions highly preferred
  • Proficiencies initiating IV therapy is a plus
  • Self-directed, empathetic, critical thinker with outstanding interpersonal skills
  • Leadership, organizational skills
  • Prior to scheduling an in-person interview, candidates may be required to complete a computer literacy and competency assessment

Additional Information
  • To be successful, you must hold a current CA RN license, BLS/CPR, and valid CA Driver’s License. Experience with IV medications and high-acuity patients is preferred.

Wage range for Registered Nurse, Level 1: – plus a 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and 401k with up to 4% match.
