About the Position:
Our leading nonprofit client in San Rafael is hiring aBilingual Patient Navigatorto join their Case Management team. This full-time, non-exempt position reports to the Case Management Program Manager and operates out of the Main Campus in San Rafael, CA.

Position Overview:
TheBilingual Patient Navigatorwill support Marin County's most vulnerable populations in accessing medical care, behavioral health care, and other essential services such as housing, shelter, and financial assistance. If you're passionate about patient care and helping underserved communities, this is the role for you. The ideal candidate will be compassionate, patient, and able to thrive in a dynamic environment.

As a Patient Navigator, you will work closely with other staff members to help patients understand their care options and connect them to appropriate services. The position involves conducting needs assessments, referring patients to services at our nonprofit client and other community resources, and providing guidance to ease patients’ concerns. A background in public health, case management, or social work is beneficial.

This role requires excellent communication skills and a proactive, customer-service-oriented approach.

Key Responsibilities:

  • Screen new and existing patients to assess their health and social service needs.
  • Respond to patient inquiries and assist with paperwork completion.
  • Provide referrals and connect patients to services at our nonprofit client and other community resources in Marin County.
  • Identify barriers to care (e.g., transportation, insurance, language) and connect patients to internal resources to help overcome these challenges.
  • Educate patients about available services and offer support to reduce anxiety and concerns.

Qualifications & Required Knowledge:

  • Compliance with OSHA requirements: ability to produce a negative TB test before employment and full COVID-19 vaccination (unless applying for an exemption), as per the August 5th State Public Order for Healthcare Facilities.
  • Compliance with the Marin County Department of Health and Human Services:
    All employees must receive the flu vaccine (unless applying for a medical or religious exemption).
  • Associate's or Bachelor's degree in a relevant field or equivalent relevant work experience.
  • 1-2 years of experience in healthcare, behavioral health, or social services.
  • Experience working with vulnerable populations, such as low-income, unhoused individuals, or those with substance abuse issues.
  • Proficiency with computers and the ability to learn new programs independently.
