Base pay range: $/yr – $/yr
Location: San Mateo County (field visits)
Coverage Area: San Mateo County
Role focus: Provide skilled evaluation, treatment and care‑planning in patients’ homes to enable safe, productive living.
- Perform home evaluations and develop individualized care plans.
- Lead and supervise Physical Therapy Assistants (PTAs) as permitted.
- Help patients improve mobility, independence, and functional performance.
- Communicate and coordinate with physicians to ensure high‑quality care.
- Document progress and report to MD and interdisciplinary team.
- Use company software for charting, documentation, and communication.
- Mile‑age/vehicle reimbursement at $0.70 per mile between visits.

- Current unrestricted CA Physical Therapist license.
- Current BLS/CPR certification.
- Valid CA driver’s license; knowledge of local community resources.
- Supervision/leadership experience preferred.
- Experience in home health and high‑acuity patients preferred.
- Strong communication, critical thinking and interpersonal skills.
- Knowledge of home‑care regulations and case‑management fundamentals desired.
- Self‑directed, empathetic, organized, and culturally aware.
