Position: Home Health Licensed Vocational Nurse (LVN) - San Marcos/New Braunfels

Home Health LVN – San Marcos/New Braunfels

Licensed Vocational Nurse (LVN) is an essential part of Home Care Providers of Texas. We prioritize our nurses and quality care. LVNs provide and document skilled nursing care per the care plan and physicians' orders, adhering to company standards and professional boundaries.

Location:

San Marcos/New Braunfels, TX.

Schedule:

Monday–Friday, 8:00 am–5:00 pm with 1 on-call weekend per month.

Responsibilities
  • Conduct skilled nursing visits according to individualized plan of care.
  • Collaborate with RN Case Manager to develop, review, and update the plan of care based on clinical findings and patient needs.
  • Deliver high-quality patient care per care plan with compassion and evidence-based practices.
  • Monitor and document patient status, report changes to RN Case Manager and/or physician.
  • Educate and support patients and families regarding plan of care, procedures, and self-care techniques.

Requirements
  • Active LVN License (Texas)
  • Active CPR Certificate
  • Home Health experience – geriatrics
  • Self-starter with organizational skills
  • Compassionate care approach
  • Valid Driver's License
