Helping People. Changing Lives. We are looking to fill two openings. One Specialist will be stationed phan's and one at 40 Prado Homeless Services Center.
Under the direction of the Access Center Supervisor, the Access Center Specialist provides direct services to individuals and families experiencing homelessness at the 40 Prado Homeless Services Center. The Specialist conducts assessments, offers information and referrals, distributes basic supplies, provides on‑site supervision, and assists with transportation as needed. This role coordinates with community service providers, hospitals, and emergency personnel to support access to housing navigation, documentation, and other services that promote stability and self‑sufficiency, in alignment with CAPSLO’s mission.
Whatyou bring to the table:
- Six months experience providing direct human or health services to the economically disadvantaged, physically/mentally disabled, substance abuse/dual diagnosed individuals, the elderly, and/or at risk youth.
- Education, volunteer experience and/or other related experience may be accepted.
- CDAC, CNA, EMT, Psych Tech, or Med Tech, or other relevant Certifications Preferable.
- HMIS experience is a plus.
- Conduct client intake, CES assessments, HMIS data entry, and maintain required records.
- Provide referrals and support clients in accessing community services and benefits.
- Assist clients with obtaining identification documents, budgeting, savings, and housing searches.
- Support implementation of Individual Plans and deliver services aligned with program goals.
- Facilitate life‑skills, independent living, and community‑integration activities.
- Monitor program site for safety and enforce program policies and procedures.
- Provide transportation to appointments and services as needed.
- Communicate client needs, behavioral concerns, and program issues to supervisors and team members.
- Complete progress notes, logs, incident reports, and other required documentation accurately and on time.
- Participate in required trainings, meetings, and assist with staff or volunteer guidance as needed.
- Follow all safety protocols and perform other duties as assigned.
