Schedule:
Monday through Friday, 8:00 AM to 4:30 PM
Highlights:
Coordinate comprehensive patient care management across acute and long-term care settings
Collaborate with interdisciplinary teams to optimize patient outcomes and satisfaction
Utilize MCG guidelines for utilization review and discharge planning
Serve as patient advocate ensuring continuity of care and resource optimization
Weekday-only schedule with no weekend or on-call requirements
Coverage for leave position with defined timeframe
Requirements:
Active California Registered Nurse license
Current BLS certification through the American Heart Association
Minimum 2 years of recent RN Case Management experience
Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings required
Demonstrated competency in MCG criteria and discharge planning across acute care environments
Strong interdisciplinary team collaboration skills
