• Develop, implement, and revise individualized care plans in coordination with the patient's physician.
• Educate patients and families on disease processes, home safety, treatment plans, and self-care techniques.
• Follow infection control standards and home health protocols to ensure patient safety.
• Provide emotional support and resources to patients and families during difficult transitions or challenging health issues.
• Travel to patients' homes within the San Fernando area, adhering to scheduled visits while managing caseload efficiently.

Candidate requirements

• 1-2 years of experience in home health or community nursing is required.
• BLS certification is required; additional certifications like OASIS or Wound Care Certification are a plus.
• Compassionate and patient-focused, with excellent bedside manner.
• Valid driver's license, reliable transportation, and current auto insurance.
