You will be responsible for comprehensive in-home assessments, developing and executing tailored care plans, and providing clinical education to patients and their caregivers. This role demands strong clinical judgment, attention to detail, and excellent interpersonal skills, as you will serve as both a provider and advocate for individuals with complex health needs, particularly the elderly or those with chronic conditions.

• Conduct in- home nursing assessments to evaluate patient needs and living conditions.
• Participate in Interdisciplinary Team ( IDT) meetings to help develop personalized care plans.
• Deliver nursing interventions as outlined in care plans, including administering medications, treatments, and tests.
• Maintain a safe and healthy home environment and ensure timely execution of physician's orders.
• Educate patients, families, and caregivers on disease management, prevention strategies, and self- care.
• Track and monitor home health visit schedules in coordination with the Home Health Manager or scheduler.
• Collaborate with physicians and IDT members to support seamless care transitions between home, hospital, and skilled nursing settings.

Candidate requirements

• Graduate of an Accredited School of Nursing .
• Unencumbered RN License in the state of California.
• Minimum of one (
  1) year of nursing experience in a clinical setting with a frail or elderly population.
• Prior home health agency experience preferred.
• Must have a reliable means of transportation to conduct home visits.
