Job specification

• Deliver direct patient care across various age groups and health conditions in a home setting.
• Conduct thorough assessments and document patient histories accurately.
• Develop and implement individualized care plans aligned with patient conditions and goals.
• Administer prescribed medications and treatments, ensuring adherence to care protocols.
• Monitor and record vital signs and patient progress; respond to any changes in condition.
• Collaborate with physicians, specialists, and interdisciplinary teams to coordinate care.
• Provide education to patients and families on disease management and prevention.
• Maintain complete and timely patient documentation in accordance with home health standards.

Candidate requirements

• Active RN license in the state of California.
• Graduate of an accredited school of professional nursing (BSN and PHN preferred).
• At least 1 year of recent experience in an acute care setting.
• For Per Diem applicants: Minimum 6 months of home health RN experience required.
• Current CPR certification.
• Valid driver's license, insured vehicle (in accordance with state/agency requirements), and a clean driving record.
• Strong communication and interpersonal skills to provide empathetic and effective care.
