Assesses (for Registered Nurse) and gathers data (Licensed Vocational Nurse) on the health status of residents/patients and provides care and treatment in accordance with physician orders and standards of practice. Responsible for performing resident skin assessments (for Registered Nurse) and evaluating/gathering data (for Licensed Vocational Nurse), obtaining treatment orders, administering treatments as ordered, and documenting appropriately to help ensure resident skin integrity is improved and/or maintained.
Essential Duties and Responsibilities- Assesses (for RN)/collects data (for LVN) for residents by physical examination including pertinent diagnostic testing to determine health status.
- Administers treatments as prescribed within standard nursing practice guidelines and in full compliance with established infection control protocols.
- Conducts and/or leads weekly wound rounds to assess resident skin condition. Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders.
- Receives, transcribes and implements new treatment orders in a timely manner.
- When requested by immediate supervisor or others in nursing leadership, evaluates a resident’s skin or makes arrangement for evaluation within 8 hours of request.
- Monitors the progress of wound healing and makes recommendations for changes as appropriate.
- Performs and records weekly pressure ulcer assessment and measurement documentation requirements.
- Documents assessments and data gathered in compliance with standards of care and company policy.
- Calculates weekly/monthly pressure ulcer rates.
- Participates in the care planning process and oversees implementation of the plan.
- Supervises Licensed Nurses (for RNs only) and nursing assistants (for RNs and LVNs).
- Communicates with physicians regarding changes in conditions, diagnostic test results, etc.
- Educates patients and their families on health-related issues.
- Graduate of an accredited school of Nursing
- Valid State licensure as an RN or LVN
- Current CPR certification
- Wound certification preferred
Supervises Licensed Nurses (for RNs only), student nurses, nurse assistants (for RNs and LVNs)
Physical Demands and EnvironmentShift work may be required. Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk and hear. Employee will occasionally assist to lift and/or transfer patients weighing between 100 to 250 pounds. Employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals. OtherRequirements
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident’s protected health information. Monitors and oversees facility’s efforts to comply with company policies, procedures and state and federal law. Salary Range$42.00 to $45.00 per hour depending on experience
