Case Manager, RN
Welbe Health PACE helps seniors remain comfortable and safe in their homes while receiving comprehensive medical care and community-based services. The Case Manager, RN will be a vital member of the interdisciplinary team (IDT), guiding participant care and ensuring high-quality, value-based services.
Key Responsibilities- Conduct nursing assessments to determine participant needs, preferences, and goals.
- Participate in IDT meetings, developing and updating personalized participant care plans.
- Deliver and document nursing interventions as agreed upon in care plans, maintaining a safe environment and promptly responding to physician orders.
- Collaborate with the PACE Physician and other IDT members to coordinate participant care needs.
- Educate participants, families, and caregivers about disease processes, self-care techniques, and prevention strategies.
- Graduate of an accredited nursing school; BSN preferred.
- Valid California RN license.
- Current Basic Life Support (BLS)/First Aid certification required; CPR and First Aid certification preferred.
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred.
- Medical, Dental, and Vision insurance.
- 17 days of personal time off (PTO), 12 holidays observed annually, and 6 sick days.

- Competitive base pay ($56.80 – $74.87 hourly) plus performance-based bonus.

Base hourly range: $56.80 – $74.87 plus bonus. Actual pay will be adjusted based on experience and qualifications.
