Astrya Global, a San Diego-based medical staffing agency, is looking for a Registered Nurse for a home health role that would provide clinical services to senior patients (65+) in Santa Clara County. Qualifications of a Registered Nurse (RN):

• Current CA, RN License

• Minimum 2 years' experience in home health (preferred)

• Driver's License

• BLS Certification

• Auto Insurance

Duties/Responsibilities of a Registered Nurse (RN):

• Conduct in-home nursing assessments for patients, primarily seniors

• Develop, implement, and update individualized plans of care in collaboration with physicians and care teams

• Perform skilled nursing services including medication management, wound care, injections, and chronic disease management

• Monitor and document vital signs, symptoms, and overall patient condition

• Maintain accurate, timely documentation in compliance with Medicare, home health, and agency guidelines
