Case Manager RN San Jose, CA

The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.

• Work a consistent Monday through Friday schedule – no weekends and no on call.
• Build relationships with participants rather than providing short‑term care.

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions as agreed upon in participants’ care plans, including maintaining a safe environment, promptly responding to physician orders, and correctly administering medications.
• Collaborate with the Welbe Health PACE Physician and other members of the IDT to coordinate participant care needs, and communicate with participants and their families to revise care plans.
• Instruct participants, their families, and caregivers regarding the disease process, self‑care techniques, and prevention strategies.

• Graduate of an accredited school of nursing;
  Bachelor of Science (BSN) preferred.
• Unencumbered California RN License.
• Current Basic Life Support (BLS)/First Aide Certification required.
• Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred.

Salary range: $56.80 - $74.87 hourly plus bonus and equity. Actual pay will be adjusted based on experience and other qualifications.
