Primary Care Provider, Nurse Practitioner / Physician Assistant - Full-Time | PACE Program

- Competitive compensation of $162,011 - $205,420 with quarterly bonus
- Generous time off - weekends off, 17 days PTO, 6 sick days, & 12 paid holidays

- Regular hours of 8am-5pm, Monday through Friday, including home visits
- Rotating on-call schedule - a few nights or weekends per month, available by phone only

We are seeking a licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly. You will work within a value-based, PACE program to provide comprehensive, holistic care and build meaningful, long-term relationships with patients. Responsibilities include delivering community-based primary care and participating in an interdisciplinary team to support patient health and wellbeing.
Qualifications- Licensed Nurse Practitioner or Physician Assistant
. - At least two years of experience working with the elderly.
- Prescriptive authority and DEA registration.
- Eligible to serve Medicare/Medi-Cal participants.
