Position Overview

Healthy Living at Home, LLC (San Jose) is searching for a professional and caring Registered Nurse Case Manager to provide case management, assessment, evaluation, direction, and care to our Home Health patients in the Greater San Joaquin County, Stockton, Lodi, and Galt, CA metropolitan area.

Essential Job Functions
  • Complete an initial assessment of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Regularly re‑evaluate patient nursing needs.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Include the patient and the family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health‑care instructions to the patient as appropriate per assessment and plan of care.
  • Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
  • Act as Case Manager when assigned by the Clinical Supervisor/Nursing Supervisor and coordinate patient care for the assigned caseload.
Basic Qualifications
  • Unencumbered Registered Nurse (RN) License.
  • Graduate of a National League for Nursing (NLN) accredited school of nursing.
Minimum Experience and Credentials
  • Minimum of one (1) year of Registered Nurse experience.
  • Minimum of one (1) year of Home Health experience.
  • Current CPR certification.
  • Licensed driver with an automobile that is insured according to state or organization requirements and in good working order.
  • Self‑directed and able to work with minimal supervision.
  • Excellent observation, problem‑solving, verbal and written communication skills.
  • Demonstrated nursing skills per competency checklist.
  • Ability to organize and prioritize workload independently.
  • Physical stamina for prolonged or considerable walking or standing; ability to lift, position, or transfer patients and lift supplies and equipment (up to 40 lb on occasion).
  • Adequate visual acuity and hearing to perform required nursing skills.
Physical Requirements

Typical functions of this role include standing, sitting, and walking a majority of the time. Occasional kneeling, stooping, bending, or reaching may be necessary. The employee must be able to lift and/or move up to 10 lbs regularly and up to 40 lbs on occasion. Specific vision abilities required include close, distance, color, peripheral, depth perception, and ability to adjust focus.
