Key Responsibilities
* ** Clinical Leadership:
** Evaluate residents and design evidence-based treatment plans, ensuring all services are approved by referring physicians.
* ** Direct Treatment:
** Deliver high-quality, hands-on therapy in accordance with the established plan of care and the California state practice act.
* ** Supervision & Mentorship:
** Oversee Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care.
* ** Precise Documentation:
** Maintain accurate evaluations, daily notes, progress reports, and recertification documentation in a timely manner.
* ** Interdisciplinary

Collaboration:

** Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.
* ** Discharge Readiness:
** Educate residents, families, and staff on safe mobility techniques and home-exercise programs to support successful transitions.
* ** Equipment Advocacy:
** Recommend and coordinate necessary durable medical equipment (DME) such as walkers, braces, or grab bars.

What You Bring
* Current, unrestricted
** Physical Therapist (PT) License
** in the state of California.
* Strong clinical reasoning and accurate documentation skills.
* The ability to motivate residents and build positive relationships with families and colleagues.
* A commitment to evidence-based practice and a passion for helping others regain their strength and independence.
