Position: LICENSED CLINICAL SOCIAL WORKER

Overview

The Licensed Clinical Social Worker (LCSW) will provide mental health assessment, diagnosis, psychotherapy treatment and crisis intervention services for adult and/or child members who present themselves with mild to moderate mental health needs. Conduct case management as needed. Collaborates with treating physician to plan and direct each patient’s treatment program through collaborative care model.

The LCSW will also participate in grant project, will co-facilitate department case consultation meeting.

Essential Job Functions
  • Provide culturally sensitive, linguistic appropriate assessment. Follow NASW code of ethics.
  • Performs a bio-psychosocial assessment interview with patient according to department policy and procedures.
  • Formulate treatment plan, conduct evidence based therapy, continue to engage in patient to improve behavioral health conditions.
  • Supports and counsels patients with psychoeducation, regarding anxiety, depression, insomnia and compliance with prescribed therapeutic regimens, including adherence to prescribed medication regimens within the scope of practice. Communicates concerns about medications or side effects to PCP/NP/psychiatrist.
  • Maintain a regular case load. Provide Warm Handoffs between primary care providers and NEMS patients.
  • Documents treatments and progress in accordance with state regulations and department procedures. Prepares reports, intakes, treatment plans, and case summaries and maintains ongoing confidential records.
  • Provides case management, referrals to other services (e.g. substance abuse treatment, specialty care, and community resources) as needed.
  • Facilitates patient engagement and provide psychoeducation.
  • Submits timely billing record to the billing department to enable billing for behavioral health services.
  • Under supervision of the Behavioral Health Manager, develops and implements both the quality improvement and outcome evaluation programs for behavioral health services.
  • Participate in grant project, will co-facilitate department case consultation meeting.
  • Performs other job duties as required by manager.
Qualifications
  • Master's degree in Social Work required from an accredited college or university.
  • Valid Licensed Clinical Social Worker license in the State of California
  • Must have a National Provider Identifier (NPI), or obtain an NPI, prior to employment start date.
  • Must be familiar with DSM-V as a means of diagnosis. Knowledge of psychiatric diagnostic classifications and corresponding effective treatment interventions supported by current research and practices.
  • Previous clinical responsibility to include crisis intervention, individual and group psychotherapy.
  • Demonstrated ability to effectively and sensitively provide care to people from different cultural groups.
  • Knowledge of social service agencies, state regulations, and professional board standards as is related to client treatment, patient rights, and client/patient confidentiality.
  • Excellent interpersonal and communication skills.
  • Minimum of two years post licensure experience in the use of psychosocial assessment and psychotherapeutic methods and measures in a hospital, clinic or agency.
  • Current documentation of Basic Life Support is required.
Language
  • Must be fluent in English and Chinese (Cantonese and/or Mandarin).
  • Other Chinese dialects or proficiency in other languages an asset.
Status
  • This is an FLSA Exempt position.
