Overview
The CalAIM Program Manager provides leadership, operations oversight, and program development for Enhanced Care Management (ECM) and Community Health Worker (CHW) services. The Program Manager supervises the ECM staff, ensures program and contract compliance, supports quality improvement, and leads the implementation of workflows, documentation standards, and reporting requirements. This role strengthens the program infrastructure, partnerships with managed care plans and community providers, and ensures services are delivered in a culturally responsive, trauma-informed manner.
BEHAVIORAL HEALTH PROGRAMS include STOP, SBEI, SBOP, Adult and Older Adult Outpatient Program, Community-based Drop-In Center, CPP, CHW, and ECM.
- Program Leadership & Supervision:
Oversee multiple program areas including ECM and CHW; provide direct supervision to ECM case managers while managing a reduced caseload; coordinate with CalAIM initiatives programs; ensure compliance with CalAIM, managed care, and agency documentation, billing, and reporting requirements; conduct weekly staff meetings, provide coaching, and monitor performance to ensure service quality; complete monthly ECM and CHW reports to meet Quality Improvement Program (QIP) goals and contractual deliverables;
collaborate with BH managers to align program goals and improve workflows. - Training, Quality, and Program Development:
Develop and deliver training on electronic health record (EHR) documentation, ECM and CHW requirements, care planning, and workflows; monitor documentation quality and support continuous improvement initiatives; organize community presentations to strengthen knowledge of local resources; participate in Partnership Health and CalAIM meetings, trainings, and community presentations; support development and implementation of Community Supports and emerging CalAIM initiatives; assist data collection and reporting. - Care Coordination & Service Delivery:
Coordinate client enrollment, referrals, care plans, appointment scheduling, and service follow-up; respond to referrals and client requests within contractual time frames; collaborate with Catholic Charities programs, managed care plans, housing partners, PCPs, and community providers to ensure continuity of care; maintain accurate, timely documentation in compliance with managed care and agency standards; assist with screenings, intakes, move-in coordination, and exit planning. - Administrative Duties:
Complete monthly reporting requirements in compliance with Partnership Health contracts and deadlines; support strategic planning and development (building workflows, shaping program structure, planning implementation); develop and distribute health education materials for clients and the community; participate in agency meetings, trainings, and cross-department collaboration; perform other duties as assigned; monitor budget, metrics, outcomes, and reporting.
Education and Experience
- Minimum of a Bachelor’s degree in counseling, psychology, sociology or a related field required; a Master’s degree in a related field preferred.
- Minimum five (5) years of Social Work/Case Management experience required.
- Minimum two (2) years of direct supervision or program oversight experience required.
- Minimum of one (1) year of home visit experience.
- Minimum of two (2) years of CalAIM experience preferred.
Required Knowledge, Skills, And Abilities
- Bilingual in English-Spanish highly preferred.
- Strong technical and relational skills; trauma-informed, culturally humble, gender-responsive, person-centered approaches; best practices in supporting survivors of violence.
- Experience providing or supervising services in complex systems (behavioral health, medical, social services); knowledge of CalAIM requirements, ECM criteria, managed care billing, and Medi-Cal documentation standards.
- Proven leadership skills, including team development, coaching, performance management, and balancing client needs with staff well-being.
- Ability to manage complex cases and navigate multiple service systems; excellent judgment, critical thinking, and problem-solving in multidisciplinary environments.
- Experience with diverse populations and implementing practices to achieve equitable outcomes; knowledge of community resources in Santa Clara County and effective service coordination.
- Strong training, facilitation, and presentation skills; effective written and verbal communication; strong organizational abilities including workflow management and meeting program deadlines; flexibility to adapt as CalAIM programming evolves.
- Experience with data collection, entry, budgeting, and organizational systems; ability to multi-task in a fast-paced environment and apply professional judgment to challenging situations; capacity to work independently within established guidelines and timelines.
