Hourly Rate: $33.75 - $43.75 per hour
Available Shift/Schedule- Full Time
- Day shift (7:00 AM – 3:30 PM)
- Evening shift (3:00 PM – 11:30 PM)
- Night shift (11:00 PM – 7:30 AM)
- 5 shifts of 8 hours
- At least one weekend day required
Responsible for providing personal care services under the direction of clinical or nursing leadership, adhering to state board regulations and facility policies. Demonstrates a positive, empathetic, and professional attitude toward patients at all times. Collaborates with the team to meet patients’ physical, biopsychosocial, developmental, and cultural needs, and addresses any unmet needs or patient complaints. Patient safety is a top priority.
EssentialFunctions
- Uses basic assessment data collection, participates in planning, executes interventions per the care or treatment plan, and contributes to the evaluation of individualized interventions.
- Provides safe medication delivery in accordance with policies, physician orders, and current practice.
- Contributes to a teaching plan related to self‑care for the patient/client.
- Ensures the well‑being of patients while providing a positive, supportive, and structured environment under the direction of a charge nurse or supervisor.
- Escalates changes in patient condition to the RN or provider to meet patient needs.
- Completes required rounds timely; observation may be assigned every 15 minutes or more frequently, up to one‑to‑one patient care.
- Obtains patient vital signs, height, and weight as assigned, ensuring accurate documentation.
- Identifies self to patient and visitors promptly and responds to requests quickly.
- Engages patients in activities and interactions designed to encourage achievement of treatment goals.
- Responds to distressed patients in a non‑threatening manner and attempts to understand the source of distress.
- Applies de‑escalation techniques to help manage patient emotions, behavior, and participation in treatment and assists with seclusion and restraints when necessary.
- Provides safety and increases coping skills of patients and assists in developing strategies for greater independence.
- Monitors behavioral improvements, reinforces these behaviors, and reports changes and responses to treatment to the team. Significant changes are reported to nurses immediately.
- Reinforces skills patients learn that promote improved physical and mental wellness.
- Leads or co‑leads individual or group sessions as part of specific recreational or therapeutic procedures.
- Participates in rehabilitation and treatment programs and teaches strategies to promote patient wellness and independence as needed.
- Oversees or assists patients with activities of daily living (toileting, bathing, dressing, grooming, oral hygiene, meals, snacks, hydration, and changing bed linens) as needed.
- Delivers care based on priority of needs and available resources to the specific service population of the facility.
- Helps with admissions and discharges, as needed.
- Provides guidance and direction to other staff members, including training of new staff or assisting in day‑to‑day operations.
- Performs day‑to‑day administrative tasks such as maintaining files and processing paperwork.
- Provides transportation for patients or coordinates transportation with appropriate staff members, as needed.
- Exhibits quality of work through accuracy, neatness, and timeliness.
- Adheres to work schedule and overall attendance.
- Demonstrates a positive, empathetic and professional attitude toward patients at all times. Addresses unmet needs, resolves complaints, and prioritizes patient safety.
