Role Overview
As a Lead Care Manager, you will play a pivotal role in managing and coordinating care services for individuals benefiting from ECM. Your focus will be on enhancing the health outcomes of high-need clients within the Medi-Cal program. You will collaborate with both internal and external stakeholders to ensure effective delivery of our services.
Key Responsibilities
- Provide guidance, support, and training to ensure the team's effectiveness in care coordination.
- Oversee the assessment, planning, and coordination of care services under the CalAIM ECM initiative.
- Collaborate with the CalAIM ECM Program Manager and service providers to optimize care coordination efforts.
- Implement and oversee quality measures and conduct regular reporting on care management activities.
- Identify opportunities for program enhancement and communicate updates to the Care Manager team.
- 1+ year of relevant work experience required.
- High school graduate or GED required; community knowledge preferred.
- Flexible hours, including occasional night/weekend work.
- Proficiency in Google Workspace, Excel, PowerPoint, Word, and internet navigation.
- Multilingual capabilities and prior experience in a related role are a plus.
At Anchor House, we believe in investing in our employees. We offer competitive hourly pay starting at $18.00 up to $22.00 per hour, along with benefits like paid time off. You'll work full-time, Monday through Friday, with on-call and weekend opportunities.
