• Provide patient care as directed on aide care plan, report and document observations and care performed
• Observe and report changes in client’s condition to Supervising Clinician
• Assist the patient in ADLs as directed on aide care plan
• Communicate patient’s supply needs to Supervising Clinician or health care team
• Display professional behavior at all times
• Comply with all areas of corporate compliance program and HIPAA regulations
• Document thoroughly and accurately in the format prescribed by the agency
• Provide companionship and patient care as directed on care plan
• Help patients and their families understand instructions given by medically trained staff

• Must have a vehicle, valid current driver’s license and automobile insurance
• Minimum 1-year experience
• High School Graduate or equivalent
• Current certification as a Certified Home Health Aide by the State of California required
• Current CPR card
• Documentation to verify work experience and legal right to work in the U.S.
• Report to work in a timely manner and adhere to call-in times for schedule changes
• Able to provide two verifiable references
