Position Overview
We are seeking an experienced, self‑driven Full‑Time Physical Therapist (PT) for Home Health in San Jose, CA. As a PT, you will evaluate patients, develop individualized treatment plans, and improve mobility, strength, and quality of life. You will collaborate closely with our Clinical Director and Clinical Staff.

Responsibilities
- Provide physical therapy services within scope and individualized plan, in coordination with the Interdisciplinary Team.
- Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, or Director of Nursing.
- Maintain accurate clinical records and submit documentation per company policy using a company‑provided device.
- Comply with all company policies and procedures to ensure high standards of care.
Requirements
- Current Physical Therapist (PT) license to practice in California – required.
- Experience in Home Health or Hospice care – 1 year required.
- Current CA driver’s license.
- Able to travel within a 15‑25 mile radius on a regular basis.
