Speech Therapist (ST) Home Health - Full Time or Part Time

Join ABL Health Care as a Speech Therapist (ST) in our Home Health team located in San Jose, CA. This full‑time or part‑time role is ideal for a compassionate, organized, and team‑oriented professional who thrives in a collaborative environment and is passionate about delivering quality care.

As a Speech Therapist, you will assess patients’ speech and communication needs, develop individualized treatment plans, and support their progress. You will work closely with the Clinical Director and team members, and manage patient care both in the field and remotely.

• Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team.
• Conduct initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
• Deliver educational information to patients and their families regarding speech and language disorders and treatment plans.
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
• Adhere to all company policies and procedures to ensure the highest standards of care.

• Current OT License to practice in California
• Experience in Home Health or Hospice care – at least 1 year
• Current CA Driver’s License
• Ability to travel within a 10–15 mile radius regularly
