• Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care and address patient needs.
• Monitor and document patient progress, adapting care plans as necessary to optimize health outcomes.

Candidate requirements

• A Bachelor of Science in Nursing (BSN) is required; an Associate Degree in Nursing (ADN) will be considered with relevant experience.
• Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or a similar setting.
• Strong clinical assessment skills and the ability to work independently.
• Excellent communication and interpersonal skills to engage effectively with patients and families.
