Location: A Woman’s Place Drop-In Center, 211 13th St.

Reports to: Director of Drop-In Services

Status: Non-exempt, Union

Hourly Wage: $30.44/hour

Hours: Monday-Friday, 9am-5pm (Temporary, up to 6 months)

Job Summary

Community Forward SF is hiring a Temporary Case Manager II. The Case Manager II assists clients with needs related to housing, coordination of care to maintain mental and physical health and personal safety, and navigation of the Homelessness Response System. They provide information, education, and referral services within a Drop‑In setting. This role is temporary, up to 6 months in duration.

Job Functions
  • Establish and maintain a caseload of clients; provide services including, but not limited to, counseling, crisis intervention, assessment, evaluation, advocacy, and co‑facilitation of groups.
  • Develop treatment plans with timelines for individual goals and objectives.
  • Meet with clients on a weekly basis to ensure successful engagement with and completion of treatment plans.
  • Ensure access to resources that help stabilize clients’ income through benefits advocacy, housing, medical status, and other needs, including social service systems and other agencies.
  • Conduct outreach to encourage program usage and make referrals to Community Forward SF Mental Health Outpatient Program or other appropriate programs based on client need.
  • Maintain client charts and associated documentation in a timely manner by successfully and effectively utilizing City electronic health record systems that may include ONE and/or Avatar, as well as successful use of G‑Suites applications.
  • Coordinate care with client service providers to ensure quality service and continuity of care.
  • Assist clients in developing external support systems.
  • Ensure that clients are treated with dignity and respect, and that their needs are adequately met, while at the facility.
  • Support our mission to promote stabilization, hope, and healing for adults experiencing homelessness in San Francisco.
  • Adhere to established policies and procedures.
  • Training and meeting attendance.
  • Other duties as assigned.
Background & Experience
  • Bachelor’s (B.A.) degree or equivalent years of work experience.
  • Previous case management experience and/or training.
  • Comfort with technology including knowledge of Microsoft Office Suite, Google Suite, and city housing databases.
  • Strong written and oral communication skills.
  • Ability to work both independently and cooperatively.
  • Ability to effectively de‑escalate conflict.
  • Thorough knowledge of homelessness, trauma, domestic/intimate partner violence, substance use disorder and recovery, HIV/AIDS, harm reduction, mental health disorders, neurodiversity, LGBTQIA+ communities, and systems of oppression.
  • Ability to work sensitively with individuals from diverse cultures, languages, abilities, backgrounds, and identities.
  • Ability to write and speak English effectively.
  • CPR and first aid certification required within 90 days of hire.
  • TB Clearance and proof of COVID‑19 vaccination required within 7 days of hire.
Preferred Qualifications
  • Multicultural, multilingual.
  • Experience with and passion for working with at‑risk client populations.
  • Lived experience with homelessness and/or other systems of oppression.
  • Strong self‑awareness, cultural humility, and commitment to personal praxis.
  • Good organizational skills.
  • Knowledge of San Francisco community resources, including the ONE Coordinated Entry system.
