Job SummaryThe Program Manager, under the supervision of the Assistant Clinical Director of Residential Programs, will oversee Swords to Plowshares’ Permanent Supportive Housing’s supportive services program at the Jon W. Paulson Veterans Community. The duties of this position include direct client case management, the provision of a robust supportive services program, and the safety and well-being of residents. The Program Manager is also responsible for implementing direct services in an effort to prevent a return to homelessness through the provision of needed support such as substance abuse and mental health services.
The Program Manager maintains an assigned caseload of veterans with various disabling conditions and provides individual counseling, group counseling, case management, assessment, advocacy, outreach, and crisis intervention. The Program Manager supervises a staff of Mental Health Specialists and a Peer Specialist and coordinates operations of the site with Property Management.
- Provide weekly supervision for a team of five staff and offer day-to-day support to ensure high-quality groups and activities are delivered to veteran residents throughout the month.
- Ensure programmatic, legal, and contract compliance, including timely and accurate reporting to achieve programmatic service and outcome objectives.
- Audit charts and physical files in compliance with agency requirements.
- Provide routine case conference facilitation with the supportive services team.
- Oversee and review case management, clinical, and peer-related service documentation of the team.
- Work in collaboration with the Program Manager for Residential Services to coordinate services and activities taking place on and off site.
- Interview and intake new residents, perform census management, and coordinate with Property Management to promote housing stability and community building.
- Attend required training to ensure individual and professional growth.
- Maintain compliance with Swords to Plowshares’ policy, federal, state, and local laws and regulations.
- Other duties as assigned.
- Provide individual counseling for veterans with mental health and substance use treatment needs, including outreach and advocacy for an assigned caseload.
- Coordinate treatment and referrals with other providers such as the VA and other medical and social services providers in San Francisco.
- Maintain appropriate and timely documentation, including progress notes and Life‑Plans to monitor progress toward goal achievement.
- Perform crisis intervention, de‑escalation, and conflict resolution.
- Engage veterans to promote personal growth through onsite groups and activities.
- Participate in weekly supervision.
- Coordinate monthly community meetings with residents and serve as a liaison with community programs such as the Institute on Aging and YMCA.
- Develop and maintain an effective interface with community resources for advocacy and linkages to services assisting tenants in achieving goals.
