Position: Residential Case Manager- 3900

3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city’s few Youth Access Points.

The Transitional Living Program (TLP) provides a stable, supportive, and developmentally appropriate residential environment for justice-involved youth ages 18–26. The program focuses on safety, healing, accountability, and growth through a low-barrier and trauma-informed approach. Youth receive individualized support aimed at improving life skills, stabilizing housing, accessing behavioral health care, completing probation requirements, and moving toward long-term independence.

The Case Manager for Justice-Involved Youth will provide comprehensive, This role provides housing education, workforce development, and trauma-informed case management to youth residing in the Transitional Living Program. The position is responsible for reducing recidivism, supporting compliance with probation requirements, strengthening community connections, and facilitating successful transitions to stable, long-term housing and self-sufficiency. Additionally, the role equips youth with the skills, resources, and guidance needed for educational and career success, fostering resilience and independence.

Duties & Responsibilities:

  • Support residents in meeting probation conditions, court requirements, and diversion agreements.
  • Welcome and engage youth as they enter the program, and conduct intake, risk/needs assessments, and ongoing case planning using trauma-informed and restorative practices.
  • Complete regular in-person check-ins 1/week minimum, including required room visits, scheduled case management meetings and drop-in hours
  • Provide crisis intervention and safety planning as needed
  • Hold a caseload of 1:20
  • Offer support and referrals related to behavioral health and substance use.
  • Resource brokerage and referral as needed
  • Case coordination and advocacy within agency and with other CBO’s
  • Participate in multidisciplinary meetings , care coordination sessions, and case conferences
  • Collaborate with probation officers, public defenders, social workers, court personnel, and community-based partners to support youth progress and compliance.
  • Work closely and consistently with on-site clinicians—including behavioral health providers, therapists, and clinical supervisors—to coordinate care plans, share relevant updates, and ensure integrated support for youth.
  • Maintain timely and accurate data collection and case notes entered into all appropriate databases.
  • Track resident progress, service utilization, and probation compliance.
  • Designing individual support & transition plans with youth including independent living skills and income stability.
  • Attend community, city, & regional housing meetings
  • Support for staff & youth in 3rdStreet programming as needed
  • Participate in staff meetings, training, and community housing or justice-system collaborations.
  • Maintain professional standards of performance, demeanor and appearance at all times
