As the Family Resource Center (FRC), family services are aimed at helping parents manage and recover from challenges, gain knowledge about parenting and child development, and develop the skills and knowledge to support their children’s learning, development and social and emotional competence.
The FRC Case Manager will need to communicate, translate, and use their bilingual skills to act as a liaison, advocate, and resource out in the community to provide intensive case management, family advocacy, and crisis intervention for families in the Tenderloin. Collaborating with other GLIDE program staff, schools, community organizations and Protective Service Workers. The FRC Case Manager also offers trauma informed family engagement and perinatal navigation.
Together, this essential role provides assessment of the family system and individual children, development of collaborative service plans, referrals to GLIDE programs and other services, and monitoring of progress towards the family’s goals. This position may periodically work non-traditional hours (early evenings, holidays, and weekends). Regular and predictable attendance is essential.
- Using their bilingual language fluency, conduct intake assessment, surveys and enrollment, collect and provide information on community resources and GLIDE services, provide advocacy and coordination to reduce barriers to services, find creative solutions to meet immediate and ongoing needs, and ensure linkage and support to promote health, recovery, and wellness for children, youth, and their families.
- Determine eligibility and review/assist with applications for entitled benefits and financial resources including but not limited to General Assistance, Cal Fresh, Medi Cal, Social Security Disability, emergency rental assistance, and low-cost housing programs.
- Develop and attend FRC outreach events and build rapport and working relationships in order to be a reliable source of information, reduce barriers to community resources, and improve awareness of GLIDE programs.
- Collaborates with other providers from other community agencies, Glide program staff, interns, and volunteers to ensure comprehensive and coordinated services.
- Facilitate and co- facilitate parent groups, specifically for expecting parents and parents with newborns.
- Lead and facilitate FRC’s weekly Mommy/Daddy & Me group.
- Deliver aforementioned services in the community, (when contracted) and school meetings.
- Manage documentation in a timely manner, which includes but not limited to enrollment, various assessment tools, individual/family service plans, follow-up contacts, CFS reports, program evaluations, and service entries in Salesforce and other external program databases.
- Participate in regular staff meetings, staff training events, supervisory meetings, accept the responsibility for aiding the development of positive team relationships as requested, and adhere to agency policies, procedures, and professional code of ethics.
- Must have fluency with reading and writing proficiency in English and Spanish in order to address the needs of the targeted population.
