Overview
Seeking a compassionate Nurse Practitioner or Physician Assistant for our Palliative Care team. The NP/PA works independently and with the interdisciplinary team to provide patient-centered care focused on symptom management and quality of life.
ResponsibilitiesResponsibilities include assessments, care plan development, regulatory visits, and facilitating hospice transitions.
Requirements- Master’s in Nursing (NP) or Bachelor’s degree and PA program completion
- Active California NP/RN or PA license
- Minimum two years’ advanced practice experience
- Strong clinical, communication, and organizational skills
- Proficiency with electronic records
- Valid driver’s license and reliable transportation
- Palliative care experience
- Board certification (NP or PA)
Position offers $59.52-$72.75/hr for 20-29 hrs/week, with benefits including health insurance, retirement plans, paid time off, and more.
