We are currently seeking a highly experienced, clinically advanced Home Health Registered Nurse (RN) – Level 3 to serve patients throughout San Francisco. In this senior-level role, you will manage complex cases with a high degree of autonomy while collaborating closely with physicians, therapists, and interdisciplinary team members to ensure exceptional patient outcomes.

- Manage complex, high‑acuity home health patients with advanced clinical needs
- Perform comprehensive nursing assessments, case management, and care coordination
- Deliver advanced skilled nursing services including medication management, wound care, chronic disease management, and patient education
- Serve as a clinical resource to the interdisciplinary team, supporting quality and compliance
- Communicate changes in patient condition to Physicians, Case Managers, and the Director of Nursing
- Maintain accurate, timely clinical documentation on company-issued devices
- Follow all company policies, clinical standards, and regulatory requirements
- Perform other duties as assigned
- Minimum 2 years of Home Health experience required; must be proficient in OASIS and case management
- 4+ years of Home Health experience preferred; expertise in complex case management strongly desired
- Ability to travel within San Francisco and surrounding areas
- Strong clinical judgment, autonomy, and communication skills
- Coverage Area: Regional Floater. Must be willing to cover a wider service area based on highest‑priority patient needs.
- Schedule: High Availability. Willingness to support weekend or urgent coverage shifts as needed.
- Schedule: Flexible. Monday–Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required.
- Pay: $72.00 – $75.00 per hour
- Annual Full‑Time Expectation: $150,000 – $156,000 per year + $10K Sign‑On BONUS
