About the Role
Reporting to and working closely with the Director of Health & Wellness, the Health & Wellness Manager ensures the delivery of high-quality clinical care in accordance with all regulatory requirements, best practices, and internal policies. This role is a hands-on, working manager who provides direct supervision and clinical support to Health & Wellness staff across all shifts, with a primary focus on assisted living.
Responsibilities
- Promote excellent resident care by training, observing, mentoring, and role-modeling best clinical practices.
- Provide hands-on clinical nursing care as needed, including medication administration, injections, wound care, catheter care, and treatments.
- Ensure compliance with RCFE clinical care regulations and maintain ongoing survey readiness.
- Monitor and enforce adherence to all clinical care policies and procedures.
- Conduct regular audits of the medication management program and escalate concerns promptly to senior leadership.
- Audit clinical documentation for accuracy, completeness, and timeliness.
- Ensure continuous staffing coverage and communicate staffing concerns to senior leadership.
- Review and approve Health & Wellness department timecards in collaboration with leadership.
- Conduct health assessments and pre-admission screenings for prospective residents.
- Lead care-plan meetings with residents and families.
- Ensure annual physician reappraisals are completed on time.
- Partner with leadership to address and resolve resident and family care concerns.
- Prepare internal incident reports, licensing documentation, and monthly tracking logs related to falls, hospitalizations, and skin integrity.
- Participate in interviewing, hiring, onboarding, and performance management of Health & Wellness staff.
- Complete employee performance evaluations in a timely and consistent manner.
- Support staff engagement, recognition, and collaboration across departments.
- Collaborate with internal and external healthcare providers to ensure continuity of care.
- Evaluate staff competency through observation, return demonstrations, and training participation.
- Support orientation and ongoing education to ensure staff compliance with required training.
- Provide coverage for the Director of Health & Wellness and Memory Care Director as needed.
- Work flexible hours, including evenings and weekends, to support 24/7 resident care.
- Perform other duties as assigned.
Qualifications
- Current California RN or LVN license (required)
- CPR / Basic Life Support certification
- Minimum three (3) years of recent nursing experience
- Supervisory or management experience strongly preferred
- Assisted Living experience preferred
- Memory Care / Dementia Care experience preferred
