• This entry-level Field Registered Nurse position is designed to develop fundamental nursing skills within a home health setting while gaining an understanding of the company's operational goals and efficiencies. It serves as a foundational role for registered nurses aspiring to advance into case management. The role works under a Case Manager Registered Nurse.

• Providing hands‑on patient care within RN license scope, including PICC lines, Wound Vacs & Care, Kangaroo & feeding pumps, management of NG, G, and J tubes, staple & suture removal, drains (JP, Biliary, Nephrostomy), Foley/suprapubic catheter insertion and troubleshooting, Pleur
  
  X drainage, trach care, blood draws, ostomy management, SQ and IM injections, IV antibiotic/TPN administration, compression wraps.
• Direct patient care in accordance with the state Nurse Practice Act.
• Implementing plans of care initiated and directed by the Case Manager RN.
• Accurate and timely documentation within 24‑48 hours, consistent with care plans.
• Assessing and providing patient and family/caregiver education.
• Assisting in skilled procedures and teaching self‑care techniques.
• Coordinating home health services and reporting interdisciplinary needs.
• Using equipment and supplies efficiently.
• Participating in personal and professional growth.
• Performing duties assigned by the Case Manager RN.
• Communicating patient status and priorities, giving timely reports to the Case Manager.
• Recording concise, relevant patient information and incidents.
• Reporting pertinent information to the Case Manager RN and Provider.
• Attending weekly Case Conferences to coordinate care.
• Maintaining patient privacy and medical record confidentiality.
• Maintaining professionalism and enhancing clinical knowledge.
• Meeting productivity standards as directed.
• Reviewing Start of Care assessments and care plans with the Case Manager.
• Participating in on‑call duties as required by weekend rotation.

• One year of recent nursing experience (waived for internal candidates/new grads).
• One year of home health care experience preferred.
• Experience with high acuity patients a plus.
• Experience working with an interdisciplinary team highly desired.
• Bachelor’s degree, accredited nursing school graduate.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Card.
• Valid CA Driver’s License with acceptable driving record.
• All licenses and certifications current at hire and sustained during employment.
• Self‑directed, critical thinker, outstanding interpersonal, organized, computer literate, excellent observation and communication skills, basic math, nursing competency.
• Must maintain compliance training and certifications annually.

Estimated Wage Range for Home Health Registered Nurse (RN): $130,000 - $160,000. Starting pay is based on relevant experience above the minimum requirements. Compensation follows a pay‑per‑visit model, with the wage provided as an estimate. 21st Century offers comprehensive health benefits and a 401k plan with up to a 4% match.
