Nurse Practitioner, Cardiology

Opportunity Information

Full-time Nurse Practitioner to join Cardiology team at 1580 Valencia Street, San Francisco, CA.

The NP will manage follow-up visits for established cardiology patients, support transitional care with post-discharge visits, and provide patient education on cardiovascular conditions.

• Conduct follow-up visits for established cardiology patients.
• Support transitional care management through post-discharge visits.
• Provide patient education and promote medication adherence.
• Collaborate with cardiologists and care teams to ensure continuity of care.
• Document patient encounters in Epic.

Graduate of an accredited Nurse Practitioner (NP) program.

• Current California NP license.
• DEA Controlled Substance Registration Certificate (CSRC).
• Current BLS and ACLS certification.
• Experience in cardiology or internal medicine preferred.
• New graduates with a strong interest in cardiovascular care are welcome to apply.
