Job Summary
Under the supervision of the UCSF Primary Care Administrative Nurse Manager, the Licensed Vocational Nurse (LVN) utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socioeconomic background, and response to medical regimen and treatment. The LVN is required to learn and apply the Patient-Centered Medical Home Standards and the Ten Building Blocks of Primary Care, and to provide nursing care to patients in the ambulatory setting.
The LVN works as a vital member of the care team, closely collaborating with the RN and physicians to deliver individualized nursing care that meets each patient’s physiological and psychosocial needs. Responsibilities include chart abstraction, administration of medications and vaccines, lab result review and monitoring, and performing procedures pertinent to pediatric and adult primary care such as ear irrigations, wound care, and urethral catheterization.
The LVN also provides patient education and must have flexibility to cover multiple UCSF medical‑center primary‑care clinic sites.

The role supports UCSF’s standards of policies and procedures for the practice, maintaining current knowledge of pharmacology and competency in medication administration. The LVN adheres to all Ambulatory Services and Medical Center policies for infection control, safety, and administrative and clinical practice, and demonstrates UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.
Additional duties include excellent customer service and communication skills on a daily basis via phone, electronic contact, mail correspondence, and personal contact with both internal and external customers. The LVN utilizes computer skills in ApeX and other UCSF computerized programs.

Required Qualifications- Minimum 1–2 years of LVN experience
- Trained and licensed to perform phlebotomy
- Experience in an outpatient primary‑care setting (Primary Care and Family Medicine)
- Excellent communication skills, problem‑solving ability, and high comfort level with learning new technology/systems
- Basic computer and keyboarding skills
- Prior experience with electronic systems/records
- Demonstrated ability to work in a fast‑paced environment
- Spanish, Chinese, or Russian language skills a plus
- Epic experience
- Assisting in minor procedures in a clinic setting
- Active Licensed Vocational Nurse licensure in the State of California
- Blood withdraw/venipuncture certification
- Active American Heart Association CPR certification
