• Support Our Clinicians: Provide essential care coordination and logistical support to field staff
• Manage Supplies: Oversee patient supply orders, tracking, and delivery
• Monitor Patient Care: Help ensure excellent clinical outcomes through vigilant care oversight
• Conduct Outreach: Perform triage and wellness calls, assisting with clinical follow-up as needed
• Manage Communications: Handle incoming clinical faxes and route them appropriately
• Coordinate Scheduling: Collaborate with the Scheduling team to meet patient care needs

• Minimum 1 Year Nursing Experience (Home Health experience preferred)
• Strong Communication

  Professional phone manner and interpersonal skills
• Tech Proficiency: Comfortable with EMR systems and Apple devices
• Highly Organized: Ability to multitask and prioritize in a high-volume office setting
• Team Player: Ready to collaborate across departments to deliver exceptional care

Monday – Friday, Day Shift

On-site at our San Francisco office (This role is not remote)
