Base pay range

$76,905.00/yr - $92,286.00/yr

Starting Salary Range: $76,905 - $92,286

Position Summary

GGRC is looking to hire a Clinical Interdisciplinary Team Nurse to perform initial medical assessment of clients to determine needs for specialized nursing care, and to participate in the development, implementation and ongoing monitoring of a care plan for the client. The Team Nurse functions by collaborating with nurses, group homes, clients, and social work case managers for the ongoing needs and support of the individuals served.

Job Location s & Travel

This position can work out of any of our offices in San Francisco, San Mateo, San Rafael and from home in a hybrid setting. Local travel within the counties of San Francisco, San Mateo and Marin is required.

Responsibilities
  • Assessment
    • Visit clients, families or group homes to determine medical needs, supports, safety issues and level of care needed.
    • Assess nursing needs, i.e., respite versus skilled care; determine level of care for community placement.
    • Assist in determining need for ongoing therapies.
    • Identify and clarify medical issues.
    • Identify medical health insurance and determine coverage and length of time needed.
  • On-going Support
    • Monitor clients on a regular basis through follow-up visits.
    • Review nursing plan, assess access to medical services and escort clients to doctor’s appointments.
    • Write reports or assessments of visits as appropriate.
    • Participate in treatment planning decisions.
    • Document and report information; coordinate with Social Worker or Intermediate Care Facility Specialists as needed.
    • Provide information to Case Managers, discharge planners and other interested parties.
    • Contribute towards crisis plans.
    • Assist with plans for preventative medical and nursing services.
    • Integrate client nursing and dental diagnostic and therapeutic plans with the dental coordinator.
  • Education/Advocacy
    • Direct family/client contact on a variety of health related issues, especially diabetes and nutrition.
    • Ensure appropriate in-service training is provided to facility staff before and during placement of clients.
    • Conduct training at group homes and day programs on diabetes, nutrition, seizures, recognizing signs and symptoms of illness.
    • Conduct training at GGRC.
    • Conduct training to Residential Services Specialists.
    • Perform community outreach.
    • Maintain and update client records according to accepted professional standards.
    • Participate in special projects and assist with additional duties or tasks as assigned.
    • Maintain all client information as confidential in compliance with HIPAA and the California Lanterman Act.
Requirements Education / Experience
  • Bachelor's Degree in Nursing
  • Public Health Nurse certification
  • Valid Registered Nurse License at time of hire and ability to maintain license in good standing
  • 2+ years in Community Nursing
