Travel RN - Case Management​/Utilization Review - Case Management

American Traveler is seeking an experienced RN Case Manager with a CA license and EPIC experience for a care coordination role including discharge planning.

Job Details

• Work in an acute care hospital environment within the Case Management unit,

• Day shifts, 5x8 hours, Friday through Monday with rotating weekends,

• No on-call responsibilities,

• Possible float to Van Ness or Davies campuses to cover sick calls or PTO,

Job Requirements

• Active CA RN license required,

• Minimum of 2 years' recent RN case management experience,

• Experience with post-acute care levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab,

• EPIC EMR experience required,

• Must reside more than 50 miles from the facility,

• Must be available to complete lab work within 3-5 days of contract signing,

• Returning travelers must have been separated from the facility for at least 1 year,

Additional Information

• Responsible for care coordination, care transitions, and discharge planning throughout the patient experience,

• Complete timely initial assessments, updated notes, and final notes,

• Coordinate with the multidisciplinary team to ensure discharge plans are on track,

• Make referrals to appropriate post-acute care providers,

• Only one holiday request for time off will be approved during the contract,
