Job DescriptionUnder general supervision, the 2920 Medical Social Worker performs routine medical social work duties; evaluates social, emotional and physical needs of clients; interviews clients and families; provides supportive counseling or crisis intervention; conducts mandatory reporting activities; advocates for clients; provides discharge planning, service coordination, psychoeducation and case management; and performs related duties as required.
The essential job functions include but are not limited to the following:
- Evaluates social, emotional, and physical needs of clients by applying social work theories; explains options and treatments and makes referrals.
- Provides supportive counseling or crisis intervention to clients and/or their significant others, families or friends; may provide referral services to therapists or mental health providers.
- Interviews clients, families, and significant others to obtain intake information, reviews financial status and screen eligibility for entitlement programs; collaborates with interdisciplinary health care teams; devises, develops, and implements treatment plans; may make a diagnosis as part of a treatment plan.
- Conducts mandatory reporting activities (CPS, APS, elder abuse, domestic violence) within required time frames and documents accordingly.
- Prepares records to document case activity including statistical reports, chart notes, and other agency forms to monitor progress.
- Advocates on behalf of clients with agencies and medical practitioners; conducts home visits when necessary to assess living situations.
- Provides discharge planning and/or service coordination; analyzes client needs and coordinates with treatment teams; refers clients to outside services and community agencies; researches community resources and facilitates follow-up care.
- Performs other related duties as required.
Minimum Qualifications
- Education:
Master’s Degree in Social Work (MSW) from an accredited program (CSWE); AND - Experience:
Six (6) months (1,000 hours) of social work experience in a health care setting (may be gained during MSW program or post-graduate). - Applicants may qualify if they are enrolled in the final term of an MSW program and will complete degree by a stated date; proof of MSW degree completion will be required prior to appointment.
- One year of full-time employment is equivalent to 2,000 hours (40-hour weeks).
- Comfort as Float responding to urgent social work requests in varied hospital outpatient units
- Experience in Urgent Care and/or Dialysis settings
- Willingness to provide case management services (on-campus only)
- Excellent communication skills
- Ability to manage multiple tasks
- Bilingual in Spanish or Cantonese

The 2920 Medical Social Worker Assessment score is banked for three years and may be reused for other job opportunities with similar assessments. The eligible list duration is 12 months and may be extended with approval.
