Responsibilities- Intervene with patients who have complex psychosocial needs and assist with eligibility determination for social programs and funding sources.
- Provide crisis intervention to patients and families with psychosocial needs and coordinate and facilitate the development of discharge plans of care.
- Coordinate resources and services in response to a patient s psychosocial needs while collaborating with Case Managers, Nursing Staff, Medical Staff, and ancillary departments to support timely and safe discharge or ongoing care within fiscal realities.
- Practice within UCSF s mission and values, participate in department education and practice improvement activities, and work in a collaborative practice model with the multidisciplinary treatment team to optimize patient outcomes and timely discharge.
- Provide social work services to patients regardless of gender, background, language, age, disability, or other protected status.
- For CSW III (LCSW), engage in advanced clinical leadership activities such as program planning, educational programs, clinical supervision, and consultation to the clinical team and community.
Required Qualifications
- Masters degree from a CSWE accredited school of Social Work.
- Experience (internship/career) in a healthcare and/or hospital setting preferred.
- Experience in crisis de-escalation and trauma-informed care.
- Knowledge of community resources relevant to illness, symptoms, mental status, and functional impairments; understanding of medical and psychological sequelae of illness.
- Demonstrates resourcefulness, effective written and oral communication, diplomacy, organization and analytical skills.
- Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume environment.
- Ability to work effectively and collaboratively with interdisciplinary teams.
- Basic pager, telephone, and computer skills including internet use.
- Flexibility to orient and work at all UCSF Medical Center locations.
- All items from Clinical Social Worker II requirements, plus:
- Licensed Clinical Social Worker in California (Board of Behavioral Sciences).
- Three years of healthcare experience (hospital setting preferred).
- Advanced clinical knowledge and skills with leadership responsibilities including program planning, education, supervision, and consultation.
- Current California Licensed Clinical Social Worker in good standing.
- Three years of healthcare experience, hospital setting preferred; pediatric experience preferred.
- Second language:
Spanish.
- Licensed Clinical Social Worker (California) / Board of Behavioral Sciences (for CSW III).
