Position SummaryThe Clinical Social Worker provides direct clinical services to individuals, families, and groups, addressing behavioral health needs with a focus on culturally responsive, community‑based care. This role will support Rafiki's integrated health model, working closely with medical providers, case managers, and community health workers to ensure holistic wellness for clients.
Key Responsibilities- Provide individual, group, and family counseling services, including assessment, treatment planning, and crisis intervention.
- Conduct psychosocial evaluations and develop culturally competent treatment plans tailored to clients' needs.
- Support clients in navigating community resources, social services, and healthcare systems.
- Maintain timely and accurate clinical documentation consistent with professional standards and funding requirements.
- Collaborate with Rafiki's multidisciplinary team to coordinate holistic care and referrals.
- Facilitate psycho educational workshops, support groups, and community healing circles.
- Provide advocacy and case management for clients dealing with housing, employment, benefits, and other social determinants of health.
- Participate in regular clinical supervision, staff meetings, and professional development.
- Uphold Rafiki's mission of advancing health equity, racial justice, and culturally grounded practices.
