IOA’s On Lok PACE program is seeking a compassionate, experienced Registered Dietitian to join our team in a per diem capacity. This role provides dependable 1–2 days of work per month, with additional opportunities to support the team during vacations, leaves, and other coverage needs.

As part of a highly collaborative interdisciplinary team, you will play a key role in assessing participants’ nutritional needs, developing individualized care plans, and ensuring high-quality meal and nutrition services for older adults in our Adult Day Health Center.

Specific Responsibilities
  • Conducts comprehensive assessments and reassessments to develop an individualized nutrition care plan for each participant.
  • Participates as an active member of the PACE Interdisciplinary Team (IDT) regarding diet and nutritional problems and management, including developing care plans, problem solving, and case review.
  • Collaborates with clinical team members to implement and monitor therapeutic diet orders and special dietary modifications.
  • Determines participants’ needs for home-delivered meal services and/or nutritional supplements and makes recommendations to the IDT.
  • Works with participants to identify nutrition-based health goals, and appropriate supports and services to promote successful completion.
  • Provides nutrition education for participants, families, primary caregivers, and/or PACE staff.
  • Ensures the nutritional adequacy and participant satisfaction of meals and snacks provided in the On Lok PACE at the IOA Adult Day Health Center.
  • Oversees ordering of Center and home-delivered meals through the contracted food vendor(s). Reviews the lunch and snack menus for adherence to participant dietary needs.
  • Provides oversight of kitchen/food areas regarding nutrition and food service standards.
  • Partners with the Food Services Coordinator to plan, coordinate, implement and document nutrition education for staff involved in meal service.
  • Assists with formulation and revision of dietary policies, procedures, and protocols.
  • Coordinates use of community resources for educational and other purposes.
  • Participates in the On Lok PACE at the IOA Quality Assurance Improvement Plan (QAIP) and Service Utilization Review processes.
  • Actively participates in and encourages actions that promote good public relations with participants, their families and friends, visitors and the community.
  • All other reasonably related jobs as assigned.
Education
  • Certified Registered Dietitian (R.D.)
Background And Experience
  • Three to five years professional experience as a Registered Dietitian
  • At least one year of professional experience working with older adults
  • Bilingual in Cantonese preferred.
  • Preferred familiarity with California Code of Regulations – Adult Day Health Care.
  • Ability to work as part of a multidisciplinary team.
  • Evidence of creativity, flexibility and professionalism.
Physical Requirements
  • Ability to lift, transfer, push/pull, maneuver and reposition 35 lbs.
  • Ability to reach, bend and walk
  • Ability to stand for long periods of time
  • Five finger dexterities
Compensation
  • Range: $43- $51 hourly

This rate is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, client needs, education and other factors.

This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
