We are seeking a dedicated and experienced Occupational Therapist to join our team in a top-tier Skilled Nursing Facility in San Francisco. The OT will be responsible for providing high-quality therapeutic services to residents, promoting independence in activities of daily living, and supporting rehabilitation in a long-term/post-acute care setting.
Key ResponsibilitiesConduct comprehensive OT evaluations and develop individualized treatment plans.
Provide therapeutic interventions focused on ADLs, functional mobility, cognition, adaptive equipment, and more.
Collaborate with the interdisciplinary team (nursing, PT, SLP, case management) to optimize resident outcomes.
Document treatment plans, progress, and outcomes according to facility and regulatory standards.
Ensure compliance with facility protocols, professional standards, and regulatory requirements.
QualificationsValid California OT license in good standing.
Minimum 1-3 years of occupational therapy experience in a SNF, long-term care, or rehabilitation setting preferred.
Strong assessment, documentation, and communication skills.
Ability to work independently and as part of a multidisciplinary team.
Contract length:
Typically 13 weeks (with potential extension).
Schedule:
Monday–Friday, 40 hours/week (as per facility schedule).
$2,400 – $3,000/week (≈ $60–$75/hour), negotiable based on experience, shift, and housing/travel stipend eligibility.
Additional benefits may include travel stipend, housing allowance, and bonus eligibility.
