About the RESET Center,operated by Connections
Located in San Francisco’s South of Market neighborhood, the RESET Center provides an alternative destination for individuals who are found to be publicly intoxicated (due to alcohol and/ordrugs) and would otherwise be transported to the emergency department or jail.
In collaboration and partnership with local law enforcement and public health agencies, the RESET Center aims to effectively divert intoxicated individuals away from the criminal justice and/or healthcare systems to improve outcomes, reduce systemic burden, and support connection to needed resources within the community.
ResponsibilitiesWhat You'll Do:
The
Case Manager at the RESET Center provides person-centered care coordination, engagement, and discharge planning to individualsutilizingthe sobering program. Working from a harm-reduction and trauma-informed approach, the CM supports participants in stabilizing safely, explores immediate needs, and connects them to ongoing services in the community.
This role conducts brief screenings and assessments, engages in crisis de-escalation when needed, and collaborates closely with community partners—including onsite Sheriff’s staff, local law enforcement, emergency departments, and outpatient providers tofacilitaterapid discharge and seamless linkage to care. The CM ensures participants leave the program with clear next steps and access to resources that promote health, recovery, and stability.
- Welcome individuals with empathy, respect, and a non-judgmental approach in a low-barrier, trauma-informed environment.
- Engage participants using motivational interviewing toidentifyneeds, goals, and readiness for services.
- Conduct brief screenings and assessments to understand risk, safety, and resource needs.
- Provide education, encouragement, and practical support tailored to participant goals.
- Develop short-term, accessible discharge plans for individuals staying between 4–24 hours.
- Coordinate referrals and warm handoffs to community resources including:
- Substance use treatment (outpatient, detox, MAT)
- Managed alcohol programs
- Mental health providers
- Shelters, crisis housing, and social services
- Hospital or emergency care when clinically appropriate
- Support crisis de-escalation and conflict intervention in collaboration with the care team.
- Document timely and accurate services entries in Epic to support care coordination, safety, and communication.
- Maintain collaborative relationships with law enforcement, public health partners, and community agencies.
- Complete required RESET Center trainings.
- Perform all other duties as assigned.
What You'll Bring:
1-3 years of experience in harm reduction, crisis services, behavioral health, or related community care settings
- High School diploma or equivalent
Experience supporting law enforcement diversion or working alongside first responders, emergency departments, or criminal-legal partners
Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire
