Essential Responsibilities

• Conduct thorough inpatient rehabilitation evaluations, including functional assessments and mobility screenings.
• Develop individualized treatment plans based on clinical findings and patient goals.
• Provide therapeutic interventions to restore mobility, reduce pain, and enhance functional independence.
• Complete IRF-PAI (Inpatient Rehabilitation Facility–Patient Assessment Instrument) documentation accurately and in compliance with regulatory standards.
• Maintain timely and detailed documentation of treatment sessions, progress notes, and patient outcomes.
• Collaborate effectively with physicians, nurses, case managers, and interdisciplinary rehabilitation team members.
• Educate patients and families regarding rehabilitation goals, home exercises, safety, and discharge planning.
• Adhere to hospital, departmental, and regulatory guidelines for patient care and safety.

• Current California Physical Therapist (PT) License.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification.
• Doctor of Physical Therapy (DPT) degree or a Bachelor’s in Physical Therapy if graduated 15+ years ago.
• Minimum of 1 year of inpatient rehabilitation experience.
• IRF-PAI experience required.
