• Location:
  
  San Francisco, CA (serving patients within a 5–10 mile territory radius)
• Population Served:
  Adult and geriatric home‑based patients
• Compensation: $130,000–$190,000 per year (DOE)

• Paid parking and bridge tolls
• Overtime eligible at per‑visit per‑diem rate

• Evaluate patient needs and develop individualized home‑based physical therapy plans.
• Deliver evidence‑based therapeutic interventions to restore mobility and reduce pain.
• Track patient progress and adjust treatment plans to support functional improvement.
• Educate patients and caregivers on exercises, safety, and mobility strategies.
• Collaborate with an interdisciplinary care team to ensure quality outcomes.
• Maintain accurate, timely documentation using user‑friendly digital tools.
• Promote patient independence and provide compassionate, supportive care.

• Physical Therapist (PT) – active, unrestricted California PT license
• Minimum of 1 year PT experience
• Valid Driver’s License and reliable transportation
• Strong communication, time‑management, and clinical reasoning skills
